Membership in the American Council for Polish Culture provides the opportunity for people to become part of a network of communication which links them to the greater Polish-American community. It enables men and women to understand their own heritage as they join in the common effort to make all Americans aware of the Polish ingredient in the modern American mosaic.

There are three categories for membership: INDIVIDUAL

• Annual Dues $10.00 or $18.00 for 2 years
• Quarterly publication, POLISH HERITAGE, is mailed directly
• Members are eligible to attend and participate in the National Convention (usually held in July or August each year)

Individual Membership Application. This form can either be filled in on-screen and printed, or printed first and filled in by hand. If the latter, please print legibly.

• Minimum membership 20
• Annual Dues $5.00/individual club member (payable at the beginning of the calendar year)
• Representation at the annual National Convention is contingent upon the fact that club dues must be received no later than 30 days prior to the Convention.
• At the Convention, the club is entitled to one delegate for every 20 members, in addition to the President
• Quarterly publication, POLISH HERITAGE, is mailed to each member

SUPPORTING ORGANIZATION

• Annual Dues $200.00
  
• One (1) voting representative at the Annual Convention
  
• Five (5) copies of each issue of the quarterly publication, POLISH HERITAGE
  
• Access to mailing list of affiliate organizations and supporting organizations
